We look forward to meeting you.  Please make special  note of the location of your appointment.   Depending on your needs, we have made arrangements to see you at either the Office  or at the Ambulatory Care Clinic.   For addresses and directions click the button below.

• Shorts and a T-shirt 
• A valid  health card
• Any relevant  X-Rays or imaging reports
• An up-to-date list of all medications you take
• A completed New Patient Health Questionnaire  (please click on the link below to download)

• Please check in 15 minutes prior to your scheduled appointment.
• The office and clinic can be busy!  If you arrive late, we may need to reschedule your appointment.
• Please ensure that you come prepared and have at least one hour to spend with us.
• If you require forms to be filled, please leave them with our assistant.  For the sake of efficiency,  forms will not be completed during your appointment, but will be returned to you promptly.
• Narcotic medications will not be prescribed for chronic conditions.  This is best managed through your family physician.
